Incident / Fire Reports, Medical Records, Ambulance Billing, Inspection Violation Requests & Environmental Site Assessment Requests
Requests for records will be filled in compliance with the Public Information Act, Texas Government Code, Chapter 552 and the Texas Health and Safety Code, Chapter 181. Open Records requests can only be submitted in writing using one of the methods below. All medical record requests must be submitted in person with a valid driver’s license and a signed medical release form. Additional information and requirements are available on the form.
